Position Title: Admissions Coordinator

Salary: $ 61,146.00 per year

Minimum Requirements:
  • Licensed Practical Nurse (LPN), Registered Nurse (R.N.), licensed Social Worker, or an associate's degree in a related field.
  • Two (2) years of experience in skilled nursing, long-term care, rehabilitation, case management, hospital discharge planning, admissions, or healthcare marketing.
  • An equivalent combination of education and experience may be substituted. 

 A current, valid driver’s license may be required for outreach activities.

It is the policy of Allegheny County that in order to receive a final offer of employment, candidates must successfully pass a background check, which includes verification of Allegheny County Real Estate Tax status, and drug/medical examinations as required for the position. The pre-employment drug screen must be completed within 48 hours of the acceptance of the conditional offer of employment.


Position Summary:

Reporting to the Regional Center Administrator, the Admissions Coordinator is responsible for coordinating all aspects of the admissions process to ensure timely, efficient, and customer-focused placement of residents into Kane Centers. This position serves as the primary liaison between referral sources, hospitals, patients, families, payers, and the interdisciplinary team to facilitate seamless transitions of care. The Admissions Coordinator evaluates referrals, verifies financial eligibility, coordinates admissions, completes required admission documentation, and provides exceptional customer service throughout the admission process. 



Overview:


Duties:

  • Coordinates the admission process from referral through admission and transition into the facility.
  • Reviews clinical referrals to determine appropriateness for admission in collaboration with nursing leadership, medical providers, and other interdisciplinary team members.
  • Provides ongoing communication with hospitals, physicians, case managers, discharge planners, accountable care organizations (ACOs), managed care organizations, hospice agencies, and other referral partners.
  • Conducts financial screenings to determine payer source, insurance benefits, eligibility, authorizations, and financial responsibility prior to admission.
  • Verifies Medicare, Medicaid, managed care, commercial insurance, hospice, and private pay coverage.
  • Collaborates with Business Office personnel regarding financial eligibility and reimbursement requirements.
  • Coordinates bed availability and appropriate placement based on resident needs, payer source, infection prevention requirements, and facility census.
  • Schedules and coordinates admissions with nursing or other departments to ensure a smooth transition.
  • Meets with newly admitted residents and/or their legal representatives to complete the admission process; reviews the facility handbook and available services with residents and family members.
  • Explains admission agreements, obtains required signatures, completes admission consent, and ensures all legal documentation is accurately executed.
  • Maintains compliance with federal, state, and the Center for Medicare and Medicaid Services (CMS)and organizational regulations governing admissions and resident rights.
  • Participates in daily census management and communicates secured admissions, pending admissions, and bed availability to facility leadership.
  • Develops and maintains positive working relationships with referral sources and community partners to support census growth.
  • Assists with quality improvement initiatives related to admissions, customer satisfaction, and referral management.
  • Participates in the facility’s on-call admissions rotation, responding to referrals and coordinating admissions during evenings, weekends, and holidays as assigned.
  • Maintains confidentiality in accordance with HIPAA and organizational policies.
  • Performs other duties as assigned by the Regional Center Administrator. 

Knowledge, Skills, and Abilities

Knowledge of:

  • Medicare, Medicaid, Managed Care, Private Pay, Hospice, and insurance authorization processes.
  • CMS regulations, long-term care admissions, and reimbursement methodologies.

Skill in:

  • Microsoft Office Suite.
  • Electronic Health Records (EHR) 

Ability to:

  • Prioritize multiple referrals in a fast-paced environment.
