- Associate's degree in Nursing AND five years of experience with Substance Use/Detox Management involving direct patient care.
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) certification is required.
- An equivalent combination of education and experience may be substituted.
Must have current CPR/BLS certification at the time of appointment. Act 33/34 clearances are required prior to appointment at the cost of the applicant.

Position Summary:
Under the direction of the Director of Substance Use Programs, the Substance Use Nurse Navigator aids the Substance Use team in providing high-quality, evidence-based care for patients with substance use disorders at Allegheny County Jail (ACJ) and helps ensure appropriate transitions of care to the community upon release to minimize gaps in care within a high-risk patient population. The incumbent works collaboratively with the Director of Nursing and Clinic Manager.
Overview:
Duties:
- Directs patient care, including clinical check-ins, injection administration, and appropriate and thorough documentation in the electronic medical record.
- Completes detailed tracking and organization of take-home doses of Medications for Opioid Use Disorder (MOUD).
- Frequently communicates with and supports Substance Use providers, administrative assistants, Substance Use Recovery Nurses (SURNs), and partnering pharmacies, providing thorough and appropriate background information to aid in clinical decision-making.
- Facilitates and coordinates post-incarceration/re-entry care of patients, including warm handoffs to community providers and social service support personnel as indicated and in coordination with Substance Use case managers and contracted re-entry support organizations.
- Schedules and tracks injections.
- Provides oversight to the Substance Use Recovery Nurses, including reviewing clinical work, charting audits, training members of the SURN team, and creating staff schedules.
- Collaborates with the Director of Substance Use Programs and Substance Use providers, recovery nurses, Substance Use Managers/Counselors, Director of Nursing and Clinic Manager, and other healthcare management.
- Performs other duties as requested or required.
Knowledge, Skills, and Abilities
Knowledge of:
- Techniques, protocols, and best practices relating to Substance Use Treatment & Services, including MOUD and Detoxification.
- Harm reduction.
- Regional clinical and social support resources related to substance use.
- the challenges that can complicate the care of individuals with substance use disorders and the stigma that individuals with substance use disorders face.
- Understanding of clinical withdrawal scales.
Skill in:
- Microsoft Office products.
- Electronic Health Records.
Ability to:
- Work with a high-risk, often under-resourced, population of incarcerated patients with substance use disorders.
- Respectfully speak with patients to support clinical staff and coordinate their care before and after release or transfer.
- Work independently while simultaneously operating in a highly collaborative environment.
