Minimum Requirements:
- A Bachelor’s Degree in Social Work, Sociology, Gerontology, Special Education, Rehabilitation Counseling, Psychology, or another Human Services Field.
- One year of supervised social work experience in a Health Care setting, working directly with individuals.

Position Summary:
The Caseworker Supervisor plans, develops, and implements the Social Service programs for residents and their families, supervises the Social Service staff, and assures utilization of appropriate community resources to meet resident and program needs. The Caseworker Supervisor also plans, organizes, supervises, and directs all administrative and operational activities and personnel of the Social Services Department in accordance with current federal, state, and local standards, guidelines, and regulations, and the facility’s established policies and procedures. This position reports directly to the Regional Center Administrator.

Duties:
- Oversees the development, implementation, supervision, and ongoing evaluation of the Social Services Department, designed to meet and assist residents in attaining or maintaining their highest practicable well-being.
- Completes and/or directs/delegates the completion of the social services component of the comprehensive assessment.
- The Social Services Director will contribute to and/or direct/delegate the contribution of social services goals and approaches to the comprehensive care plan.
- Facilitates residents’ safe transition back into the community through interdisciplinary discharge planning and arrangement of community-based services and follow-up care.
- Coordinates the implementation and oversight of procedures to ensure social services actions and interactions are adequately documented in each resident’s medical record, and that legal, ethical, and professional standards of social work practice are being upheld in written recordings.
- Oversee the process of Advanced Care Planning for each resident upon admission.
- Manages the resident grievance/concern process and assists residents in voicing and obtaining resolutions to grievances, ensuring follow-through and that the resolution has been completed as per facility policy.
- Ensures that staff members are knowledgeable about Residents’ Rights and encourages staff to maintain and enhance each resident’s dignity in recognition of each resident’s individuality.
- Advocates for residents assist them in asserting their rights within the facility.
- Directs the Behavioral Health and Psychology program and ensures that residents who display mental illness or psychosocial difficulties, such as coping with grief and loss, will have access to appropriate treatment and resources.
- Supervises caseworkers and beauticians in implementing care plans and in developing preliminary and comprehensive assessments of social service needs of residents.
- Participates in Regional Center Federal/State inspection surveys and assists in developing and implementing a plan of correction.
- Leads Resident and/or Family Council.
- Performs related tasks and duties as assigned by the Regional Center Administrator.
Knowledge, Skills, and Abilities
Knowledge of:
- Principles of and best practices of supervision and people management.
- Laws, regulations and guidelines that govern Skilled Nursing and Long-term Care facilities.
- Case management/social services.
Skill in:
- Microsoft Office Suite
- Electronic Health Records (EHR)
- Basic computer skills
Ability to:
- Make independent decisions when circumstances warrant such action.
- Deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
