Minimum Requirements:
- High School Diploma or equivalent AND four (4) years of criminal justice or social services experience.
- An equivalent combination of education and experience may be substituted.
Act 33/34 clearances are required prior to appointment at the applicant's cost. The applicant must be 18 years of age or older and maintain a valid driver's license, car insurance, and access to a vehicle.

Position Summary:
The Re-Entry Services Coordinator (RSC) is responsible for creating, managing, and coordinating the service plans of incarcerated individuals involved in Re-Entry Programming. This position's responsibilities are focused on facilitating re-entry to the community (Phase II) after incarcerated individuals meet Phase I program requirements. The Re-Entry Services Coordinator reports directly to the Re-Entry Services Supervisor and collaborates with uniform and non-uniform staff at the Allegheny County Jail. Incumbent works throughout various locations within the Jail, often interacting with Correctional Officers and other Alternative Housing program staff. The Re-Entry Services Coordinator works across the daylight and evening shifts Monday through Friday. A weekend day may be required on a case-by-case basis.
Overview:
Duties:
- Travels within Allegheny County to the client’s place of work, residence, or Allegheny County Probation Office to facilitate client meetings.
- Schedules a Warm Hand-Off meeting with the RSC Supervisor prior to the Phase II meeting; provides the client with their contact information and instructs them to reach out for assistance.
- Engages with the client's natural support(s) within 30 days of the warm hand-off and includes the family in the client's transition back into the community.
- Attends the Phase II meeting with all service providers in the Re-Entry program.
- Utilizes the Allegheny County Jail Collaborative (ACJC) to make referrals for post-release in order to track services and develop the Phase II plan; meets with the client while in the Allegheny County Jail (ACJ) or alternative housing at least twice a month to continue developing their relationship and to work on the client's community plan.
- Collaborates directly with the client, their natural support(s), Allegheny County Probation, Alternative Housing, and service providers to develop and implement a Phase II community service plan for each participant.
- Completes all required parole paperwork, which will be submitted to the assigned Jail Caseworker or alternative housing personnel at least 14 days prior to the client's minimum release date.
- Adheres to the contact guidelines outlined in the established reentry program model, starting with the initial phone call within 48 hours post-release and setting up the first face-to-face appointment within 72 hours.
- Coordinates with the Allegheny County Probation to make the first face-to-face appointment. This joint meeting will provide the client with a copy of their Phase II Plan and a resource packet.
- Acts as an advocate for the client to ensure they can successfully navigate their re-entry journey and coaches the client in self-advocacy and resiliency while offering ongoing encouragement and support.
- Performs other duties as assigned.
