Minimum Requirements:
  • Graduation from an accredited nursing program and three (3) years of long-term care experience. Applicants must possess a current license to practice as an RN or LPN in the State of Pennsylvania or must possess a current temporary practice permit authorized by the State of Pennsylvania, or  
  • Two (2) years of casework or social work experience in a long-term care or medical facility, or
  • An equivalent combination of education and experience may be substituted. 

 It is the policy of Allegheny County that in order to receive a final offer of employment, candidates must successfully pass a background check, which includes verification of Allegheny County Real Estate Tax status, and drug/medical examinations as required for the position. The pre-employment drug screen must be completed within 48 hours of the acceptance of the conditional offer of employment.


Position Summary:

The Admissions Coordinator is responsible for the pre-admission nursing assessment of applicants prior to admission to the Kane Community Living Centers. Assessments are conducted either in person or by telephone ensuring proper placement of applicants into the Kane Community Living Centers in accordance with Federal, state, and local standards, guidelines, and regulations that govern long-term care facilities to assure that high quality care is provided to all residents. Applications are reviewed on location of placement, availability of desirable community living center, acuity level of placement, and availability of beds.



Overview:

The Kane Community Living Centers are a seven-day, twenty-four-hour operation. This position may be required to work overtime as determined by management to meet operational needs. The incumbent of this position is also subject to call back during emergency conditions, as determined by management.


Duties:

  • Conducts thorough assessment of applicant from phone inquiries and onsite assessment of applicant when appropriate.
  • Provides specific information regarding the admission process or status to families, consumers, community hospitals, community agencies, etc.
  • Reviews application forms and completes pre-admission assessment form determining appropriateness for admission, and coordinates admission to regional centers.
  • Monitors Intra-Regional transfer lists and arranges transfers.
  • Contacts insurance representatives when indicated confirming authorization for admission and medical equipment.
  • Coordinates Regional Centers’ community hospital returns.
  • Maintains statistical records.
  • Assists Allegheny County Area on Aging with emergency and respite admissions.
  • Communicates with outside agencies, facilities, etc. such as Medical Assistance Eligibility Verification System and Community Hospital Medical Records Department to verify hospital stays.
  • Participates in community activities such as Allegheny County Career Fair, Senior Fair, and Stroke Awareness Programs.
  • Performs related duties as assigned or directed by the Admission Services Manager.

Knowledge, Skills, and Abilities

Knowledge of:

  • Laws, regulations, and guidelines that govern long-term care.
  • Medicare, HMOs, Medical Assistance, and third-party insurance.  

Skill in:

  • Microsoft Office Suite

Ability to:

  • Relate to and work with ill, disabled, elderly, and emotionally upset people within a long-term care facility.
  • Plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, related to the admissions policy of the Kane Community Living Centers.
  • Operate a personal computer, photocopier, and fax machine.
  • Communicate effectively both orally and in writing with co-workers, residents, family members, visitor’s government agencies and the general public.

Residency: Allegheny County within 1 year.
