Position Title: Community Coordinator - Family & Child Health

Salary: Hourly: $24.0385 || Annual: $50,000.00

Minimum Requirements:

Bachelor’s Degree in public health, social work or a related health field and three (3) years of community engagement experience and one (1) year of supervisory experience


OR


Master’s Degree in public health, social work or a related health field and two (2) years of community engagement experiences and one (1) year of supervisory experience


OR


Any equivalent combination of experience and training



AND

Pre-Employment Proficiency Exam

Computer Literacy Exam – Measures basic computer skills, basic knowledge of Microsoft Excel and Outlook, and typing. Non-ACHD applicants must pass a computer literacy examination administered by the Allegheny County Health Department with a score of 75% or higher.

Condition of Employment

It is the policy of Allegheny County that in order to receive a final offer of employment, candidates for positions within the executive branch must provide proof of a Covid-19 vaccination. The prospective employee will need to be fully vaccinated or have at least one shot of a two-shot series before beginning work. Any employee who does not receive and provide proof of the second shot within 30 days will have their probationary employment terminated. Allegheny County will allow for medical and religious exemptions from the requirements in accordance with state and federal law. 

Additionally, it is the policy of Allegheny County that in order to receive a final offer of employment, candidates must successfully pass a background check, which includes verification of Allegheny County Real Estate Tax status, and drug/medical examinations as required for the position. The pre-employment drug screen must be completed within 48 hours of the acceptance of the conditional offer of employment.

Purpose

To establish open competitive and promotional eligible lists for use in making merit system appointments in the Allegheny County Health Department. The list will contain the names of successful candidates ranked in the order of final earned ratings. The list will be in effect for one (1) year unless previously exhausted or superseded.

In the ranking, open competitive applicants qualifying for Veteran’s Preference receive an additional ten points on the final earned rating. Form DD-214 must be attached to the application form for consideration for Veteran’s Preference. Determination of Veteran’s Preference is made in accordance with State Civil Service Policy.

Position Summary:

The Allegheny County Health Department is hiring a full-time Community Coordinator who will be responsible for the planning, directing, coordinating, and management of community engagement efforts for the Office of Family & Child Health (FCH). The Community Coordinator primarily oversees program enrollment, resource management and coordination, community partner relationship maintenance, and represents the program by leading or collaborating on Maternal Child Health focused initiatives. Additional tasks include identifying and supporting community partnerships, reporting, strategic planning, and project management. The Community Coordinator in this program works under supervision of a public health administrator.




Duties:
  • Maintain regular communication with partners and community organizations
  • Seek opportunities to collaborative with organizations with similar maternal child health related goals and/or similar priority populations served
  • Regularly audit program enrollment data
  • Train and support enrollment staff
  • Maintain accurate contact and service information for referral sources
  • Conduct outreach to the community (individually, and as a team member) to build strong relationships with community members, organizations, businesses, and community groups
  • Manage enrollment and referral processes
  • Regularly communicate community resources/services with FCH staff
  • Work collaborative with Public Health Educator and Fatherhood Engagement Specialist to coordinate outreach, education, and community facing events and related efforts
  • Developing, coordinating, and implementing community facing efforts that are timely and relevant to the needs and desires of the community
  • Coordinate and host regular community meetings (outreach planning, community facing education, and other related topics)
  • Manage maternal and child health collaborations/collaboratives, as well as represent the program at advisory committees and other related meetings
  • Establishing and maintaining processes to maintain warm handoffs/connections to services for families as well as program staff
  • Track and maintain supply inventory
  • Coordinate efforts to ensure outreach team has the tools and skills they need to effectively participate in quality community engagement
  • Contribute to the development of MCH Program goals and objectives through participation in leadership meetings, coalitions, related program strategic planning, and other efforts
  • Lead coordination of FCH program family events
  • Continue self-education of quantitative and qualitative health information, particularly related to maternal and child health
  • Ensure utilization of a range of approaches to communicate health information to various audiences (e.g., community members and stakeholders, staff, students) with culturally relevant and linguistic proficiency
  • Participating in evaluation of programs and using the results to improve program and organizational performance
  • Keep record of all active partners and referring organizations in case management software
  • Submit managed care organizations monthly reports to PH Administrator
  • Complete quarterly community engagement tracking report
  • Submit supply orders for community engagement and partner distribution materials
